Any country that is on the receiving end of our military abilities would not be able to distinguish it from war that had been declared by Congress, so the official declaration seems to be a moot point to me.
Any country that is not controlled by the central banks and has one or more resources that our corporations desire is on the list. They may not all get coverage, but if active duty US troops on the ground or killing from the air counts, we have unofficially declared war on:
United States (NDAA, Patriot Acts, Military Commission Act - I guess this war has been more officially declared than any of the others)
Lakota
Afghanistan
Iraq
Pakistan
Palestine (by proxy and sanctions)
Syria
Korea
Yemen
Mexico
Iran
Africa (Yes I know it's a continent, but the list of African countries would be close)
South America (see above)
Central America (exclude Panama, maybe)
I have probably missed some, but this is a good start.

According to the American Constitution we are not at war unless Congress declares war and Congress has not declared War since WW2.
