Should nurses follow policy or protocol?

I heard someone say that nurses in my hospital can only stick a patient twice (for an IV) then they need to get someone else. Someone told me that "it's basic protocol but NOT a stated POLICY that a nurse can only stick a patient twice."

Does that make sence? I always thought they were 2 very different things, and IV sticks wouldn't be part of a hospital policy anyways, more like the policy is for nurses to follow their nursing protocols, and there protocols state no more than 2 sticks. Am I wrong? Can you explain it? Is it more important to follow protocol or policy?

    This is all about patient safety and good patient care.

    In my hospital there is an IV team...nurses who specailize in placing IV's.

    Now if a nurse who has been doing this for YEARS cannot find a vein on two sticks...she/he will peer refer....if that peer cannot get a vein in two sticks then the patient is MD referred...MDs in general are not good at placing peripheral IV's...so they usually will write an order that it is okay to place in the foot or for a PICC or other central line.
