How in the hell do you get the hero's journal entry in Call of Duty World at War?

I just got to the last mission. I have played through the campain on Regular&Hardened, getting the neutral entry each time. The first 2 times, I thought I did every heroic choice. I never shot the 3 enemy soldiers who are fatally wounded when you get rescued by Reznov, I didn't shoot the Germans who were surrendering by the Subway, I took every chance I could to spare any enemies I could. I read online after I completed it on Hardened that you can't throw the molotovs at the Germans the first level you get to use them. I was mad at Activision for not communicating that clear enough, but I was fine. For good measure, I didn't use the molotovs at all in the Soviet campaign. And it is Goddamn hard to complete the game without using your molotovs. Somehow I did it, I got to the final mission about a few minutes ago. Then those familiar words from Chernov lit a stick of dynamite in my soul, I was so enraged. I'm almost on the verge of furious tears. So enraged that I immediately reset my whole Veteran campaign to a new game, almost to tell the game that I will not be denied. Must've taken me 3 weeks to beat the whole game on Veteran, and now I have to go back to square one. I reset it because I wanted to play the Veteran campaign once as the faultless hero, and once as the Red Army monster that Chernov describes in said ending. I like to consider myself an intelligent gamer, and story really counts to me. How in the hell does Activision expect us to know what to do as the hero?
