Do conservatives understand that programs like public education, medicare, and social security are what makes America a first world nation?

They don't care! They are hell bent on creating a third world nation within the US! Been their agenda since they started selling out our industrial base in the 1970s!

The difference between a country like the US and one like, say, El Salvador, is not the amount of wealth but the distribution of wealth. This is not about -conservatism-, it's about the Republican Party. They want to concentrate wealth more and more, until American workers will work as cheaply as workers in Nicaragua or Bangladesh, so they don't have to go overseas to hire super-cheap labor.

And for more than 40 years now they've been moving us in that direction. Since just before Reagan got elected, the middle class has continuously lost ground while the richest 1% have doubled their wealth, and their share of ownership, every decade.

This is also Donald Trump's fond hope. Cheaper labor means more profit, and profit is the Republicans' God. Return on investment. The creation of wealth for the wealthy. A third world nation is more profitable, therefore it is God's will!
