What kind of restaurants to take a woman you really like, very fancy or casual?

In the middle, not McDonald's, but not somewhere I have to wear heels if I don't want.

It's up to you but if you're taking her to an upscale restaurant, let her know so she can dress appropriately.

I enjoy fine dining myself. I always opt for fancy. Occasionally this is too intimidating, but more often, it seems to give them a feeling that they need to reciprocate with sex later in the evening.
