Gallbladder attack for me. It was so painful I had to hit myself to get a different type of pain.

Kidney stones.
A very close second was tooth extraction without anesthesia.

Rookie dentist failed to put anesthesia before my two wisdom teeth were removed and during the process also damaged a nerve. Extraction hurt but Healing process was a b-word

Broke my ankle in three places when I slipped on the stairs and landed on a marble floor. Ouch!
In the end, healing from the surgery was more painful than the actual event.
