Unsure what to put for my 'separation reason' on unemployment?

So I know the expanded PUA eligibility to include "Individuals who refuse to return to work that is unsafe or to accept an offer of new work that is unsafe". Im trying to file right now but the process is so confusing.. I'm opening a new claim labeled extension claim on the site but can't tell if its PUA or just regular unemployment, and when its asking me for my reason of seperation from my job it's not listing anything covid related. I don't think it would be temporary close/shut down because they're still open at reduced hours. The closest seems to be leave of absence but Im confused about the process and worried putting that would exclude me from benefits? Tried scheduling a call appointment w unemployment office but they're fully booked.. currently on hold with them hoping to get somewhere..

Update:

in michigan

Update 2:

also will only be gone from my current job two weeks at most... not just "scared to work" two coworkers just tested positive w covid and my job is not taking any precautions + unable to social distance but thank you for being condescending. i literally just need to know what to put as the reason for leaving

We don't know what state you are in.

Unless you have an existing claim, you open a BRAND NEW CLAIM not an extension.

However, it's possible that a NEW CLAIM can't be open for I'm afraid to work since the pandemic has been going on more than a year and you were working this full time.
