I think they look nice. I did the calligraphy for all of my envelopes (I think I had about 80 of them). I loved the way they turned out. I was extremely flattered when my grandpa's cousin complimented me on them, because frankly I didn't expect too many people to take note of my hard work.
I briefly learned a bit of calligraphy when I was in high school, and my now-husband bought me a calligraphy set and "The Complete Idiot's Guide to Calligraphy" so that I could practice. I had a lot of fun doing it.
As for the average cost, it depends on whether you get an amateur or a professional. I think $1-3 dollars per envelope is the going rate.

I received a beautiful calligraphied wedding invitation from a close friend's family. I asked her who did the lovely job of writing them. She said believe it or not it was a computer program they bought which printed it on the envelopes. It was perfect and looked hand done. I have a computer print shop disc myself which does not do that in particular but it only cost about $40.

I love beautifully addressed envelopes, calligraphy or beautiful handwriting, either one.
But it really doesn't make any difference to me other than when I first see it and notice the attention to detail of the sender.
Is it worth it? Not to me if I have someone who will hand write the addresses neatly and attractively.

I think for very small weddings (like no more than 30 people) it makes sense, but for bigger weddings, I think a lot more effort would go into it than is actually appreciated. I received a wedding invite last week, the envelope was hand written, and it is in the bin.
I remember seeing somewhere (can't remember now, but online) where they incorporated the white typed label into the design of the envelope.
