Following your heart is generally translated into "be brave and do what you want to. don't hold back". Being bold and daring will get you to your desired place. For some, that means finding themselves.

trust your gut feeling, your instincts. for you to "follow your heart" translates into knowing and trusting your core being. the path you take and decisions you make will be as of a result of your values/morals/experiences thus leading you back to yourself.
