If a child has reduced feeding and dry nappies- DONT wait any longer- children get dehydrated very quickly and the consequences are a lot more serious. he is lethargic and has a fever- he possibly has an infection requiring antibiotics so waiting could make the infection get more serious.
calling NHS direct 0845 46 47 is a good idea if you want to double check but you could equally go straight to a&e.

lethargic is a definite sign that he needs to be seen by Dr. They will be more sleepy when they are sick but being lethargic is a big warning sign. He also sounds like he is not getting enough fluids, possibly dehydrated. I would take him now.
good luck
