Why does my steering wheel do this?
I have a 91 explorer and sometimes when I turn the steering wheel, it makes a clunking sound that sounds like its coming from inside it and it really doesn't sound good at all but I'm just wondering if it is something really bad or what?

Does it happen when you turn in both directions, or just one? If it makes the noise in both directions it could be your steering column. If it only happens in one direction or the other, it's probably a CV (constant velocity) joint or a tie rod end going bad.
Either way, it is not a safe condition and you should have it checked out by a qualified mechanic.

It's coming, probably, from your steering column. They get noisy when they get older. You can fix this by dismantling it, cleaning and lubricating it. It's nothing that would affect the functionality of the vehicle...if the problem is indeed the steering column.

yes it's bad and can be dangerous you have apart in the Calum that let's you tilt the steering wheel that is the part that's going out and you would have to replace the hold steering calum which you can pick up at any scrap yard.
