Pregnancy on the pill?

If the combination pill's job is to prevent ovulation and assuming the pill is taken as prescribed, why is ovulation occurring in that approximate 1% of the population? Are estrogen and progesterone concentrations not high enough in modern pills? Did manufacturers of modern pills gamble on the sake of less side effects accompanied by a higher probability for pregnancy?

I have heard that in some instances, a woman's weight may be a factor. Specifically, use of the birth control PATCH is not recommended for over weight women presumably b/c there are not enough hormones contained in the patch. A heavier woman would need more of the hormones. I don't think the manufacturers are gambling with effectiveness rates, but there are just so many variables to consider that nothing short of abstinence will be 100% effective in preventing pregnancy.
