When will my breast milk production increase?

My baby was born a month early, naturally and by her choice. Because she was so early she is not able to latch on properly for feedings. We purchased a pump and I have been pumping several times a day but have not been able to keep up with her, we have been supplementing with formula to make sure she is getting what she needs. I am well hydrated and eating just as well as I did while pregnant. She is now three weeks old and I want to keep her on as much breast milk as possible and would like to have this under better control before I return to work in three weeks. Thanks for your help!

Already seen a specialist and they tell me to keep doing what I am doing. We continue to try to get her to latch on but she is still not able to do so effectively enough. I have inverted nipples as well so that only makes the process more difficult. We purchased a nipple shield and still have not been successful enough. I have to pump, the question is when will my production increase? It has remained the same regardless of how many times I pump and I pump as often as she eats.

    I agree you need to talk to someone that can help with latching. Your child is old enough now. Pumping is good to stimulate production but baby is better. You should be putting baby to breast first and when you are done the pump to let your breast know they need to produce more. If you are seriously lacking milk there are some herbal things you can take like blessed thistle or see your doctor and get a prescription of Domperidone. I took that from about 6 months on as I just wasn't producing enough.

    Relax, breastfeed often and seek some help if you need it. It is hard to stay focused and keeping your supply up when supplementing.

    Just read your update. It is hard to tell what your supply is like when pumping. Pumping can only stimulate so much and not at all as effective as having baby to breast.

    I think you should speak to someone from La Leche Leauge. Just because you have inverted nipples may prove more difficult but can be done with proper guidance. Help is out there you just have to find the right person.

    Pump / feed every 2 hours, go up to 3 hours at night. That is from start time to start time. You should be able to get in 10 - 12 pumps a day.

    Do this for the next 3 weeks if possible. IF not , go to 3 hours during the day and up to 4 at night.

    Bottles sounds fine in your case, you can do breastfeeding as well. Just keep trying and use playtex slow flow wide nipples. They are really helpful to prevent nipple confusion.

    "Pumping several times a day" is likely not enough to establish good milk production. You really need to be doing it at LEAST 8 times a day, and aiming for up to 12. You can lessen the amount later.

    By several times a day I'm worried your just doing it 3 or 4 times a day, kind of clusetered together, and that won't work. Not even close to ever working for anyone really. Not to get production started. You really need evenly spaced and aim for 8-12... 10 - 12 being ideal.

How often you pump absolutely makes a difference in the total amount you will get in a day. It isn't like it makes the a difference that day necessarily, but the more empty your breasts are over time, the quicker you will make milk. So to make more milk quicker, you need to have them empty more often. More empty = more milk, and emptier = more milk. But you don't need to drain them more than every 2 hours. That is really going to maximize your production without making you sore or insane. Let the pump work for 15 minutes at a time (a double electric pump). At least 5 minutes, otherwise you likely are not getting letdown. After 15 minutes if you haven't seen any more milk (or very little) for a couple minutes you can end the pumping session. In the hospital some lactation consultants were FAR mroe helpful then others. Some were pretty useless to me, and some fixed issues I was having.

    Yes, you need to get breast feeding well established before you return to work.

    Try this. Take two days where you can have friends or relatives help you - spend two days resting, in bed if you can, and breast-feed your baby as often and for as long as you can. Drink plenty, sleep as much as you can - you need to try to be well rested and nursing as often as possible. The more your baby sucks and stimulates your breasts, the more milk you will produce.

    Have someone else do the cooking & cleaning, someone else can do the shopping and laundry - take two full days to just rest, drink lots of liquids and feed your baby as often as she will eat. Pump after each feed, this way your breasts will produce more (they will think your baby is still drinking).

    EDIT: Sometimes small babies can't latch on well because the breast is too full of milk, so you could try expressing a little milk from the breast BEFORE you put the baby on there, it will make it a little easier for her to latch on.

    Honestly, pumping will never be as good as suckling from the baby. Try to get her to latch now that's getting older. Go see a lactation specialist.
