Cat pee accident...always a UTI or illness?

Likeliest thing that happened was that they were in the box but hiked their butts up too high and peed out of the box that way. When this happens here we lower the litter amount to about 1" at most, to get their butts down further. It does help a lot. Since yours was happening right next to the box, I'd figure they were too close to the edge and it just went over.

But you should be aware that there are many reasons for a cat to urinate inappropriately. Some and most common is actually behavioral.
