How to fill out a W2?
My father is deceased and never did (any) W2's for a woman who assisted him for a medical condition in his home. The IRS & Social Security are pestering me to file a W2 for 2009. Here's what I know, 1. father's EIN # 2. Woman's name and social security # 3. Amount she was paid (there were no deductions)
QUESTIONS
1. Who's address do I put for my father? c/o me as this is where his mail is coming anyway?
2. I do not have an add for the employee, can I leave this blank?
3. Apart from line 1. "Wages, tips, other compensation" and line 3. "Social security wages" do I need to fill in any of the other blocks?
4. Do I need to fill a W3?

For 2009 simply fill out a 1099-Misc showing the total amount paid. Send it to the IRS. Put down the address of the cemetary where your father resides?

there are several problems here
you indicate no taxes were withheld which would indicate your father was paying her as an independent contractor and she would need to be issued a 1099
however you indicate he had an EIN which would mean he was paying people who worked for him as employees, where he withheld taxes, submitted and paid quarterly report taxes during 2009
you probably need to search your father's records to see if he properly filed the quarterly reports(or if he was claiming this person as a household worker, the taxes paid on Sch H
but even at that the payment to the employee should have had at least FICA taxes withheld from the employee's pay
