Does this sound like I have a concussion?

Today during game one of a baseball doubleheader I attempted to steal second. It was a close play and I slid feet first. The second basemens knee and the left side of my head collided at full speed. My helmet went flying and I lay there for about a minute. It basically felt like someone clocked me in the head really hard. My coach came over to me and asked me a bunch of questions like where are you? I passed and it took about an inning for me to feel better. However on my ride home, I started to get a pounding headache and felt a bit drowsy. Moving my head is a bit uncomfortable and seems to make the headache worse. I tried lightly shaking my head which feels weird, like something is loose inside it. I also have a knot on the side of my face next to my eye. Does any of this sound like a concussion?
