How to file complaint against Skilled Nursing Facility?

I live in the Los Angeles area and want to file a complaint against the Skilled Nursing place my 93 year old mom was at. Its not abuse. I was forced to remove her way before her Insurance ran out. I found out this is illegal. She was forced out after only 2 weeks while waiting for her broken hip surgery to heal. What website should I use? Thank you.

    Nursing homes are required to issue discharge letters to residents. The usual discharge time period for notice they are required to give is 30 days; however, nursing homes can issue emergency discharges to residents.......was there a discharge letter provided to you? It is important to gather more information, because you could make a report right now to the state...but this isn't going to be something they are going to go into a facility for to do a complaint investigation in a quick turnaround. When states get complaints, they triage them due to severity.....if they have already been in the nursing home recently for an inspection and the information they received is not life threatening or caused harm they may not investigate it until they get other complaints that take them into the nursing home (could be months later).

    Usually nursing homes want your business, especially for physical therapy because they make a lot of money. So my assumption is that there sounds like there are other factors surrounding what happened.

    Also, keep in mind that Medicare will not pay for therapy if the physical therapist assessment shows that there has been no improvement, or the resident is unable to do the exercises or refuses to do so. In those situations, nursing homes are required to issue a Medicare notification letter to the resident/and or family representative.

    If you are your mother's durable power of attorney for health care, my thoughts are to go ahead and request a copy of her medical records. It won't cost that much money since she wasn't there that long. The more information you can provide to the state agency that investigates complaints in nursing home, the better they are equipped to know details of what happened.

    my mom had an incident in a nursing home a week ago and nobody found out not until the next day when my mom was asking for a pain medication & complaint of a big bump on her head. Nobody did a incident report the night when the incident happened. They didn t call the ambulance or paramedic too not until I came & told them what to do. Which website do I go to file a complaint against this facility?
