I'm diabetic and my numbers are mostly good but there's been 3 days this month where it was 140 or 141. What causes that to happen?

Who knows. This is why doctors do not look at the numbers except to look at trends. They’re more concerned with your A1c which is the metric of your average.
Odd spikes happen, as long as they don’t happen too often you’re likely fine.

When were you testing that you were getting those numbers? Is that fasting? Postprandial? Random?

Eating the wrong things.
The body responds to foods by releasing a certain amount of insulin. Certain things that you think are food are actually causing liver distress that prevents the brain from ever feeling full, so your brain both ramps up the hunger (binge eating) AND tells the pancreas to increase the amount of insulin production, which leads to the pancreas failing (diabetes II) and is a process known as insulin resistance.
In short, your body is not storing and using what you eat properly, especially when consuming sugar, HFCS, honey, nectar, juice, molasses, maple syrup and/or white flour products (most breads, crackers and pastas).
This overeating and misuse leads to the accumulation of glucose in the blood (141, 140).
