Is a therapist obligated to follow through on a referral?
I was seeing a therapist for 3 months, and it was a mutual decision to end the sessions as nothing was working with the issue that I have. She stated that she could refer me to another therapist that specializes in a different kind of therapy that she did not specialize in. I agreed. A week later, I still have not heard from her. Is she obligated to follow through on this referral?

Call her and remind her.
Perhaps she had to wait and call the therapist she was referring you to and see if he or she was taking new patients. Also, you should have signed a release of your medical records to this new therapist right in the office before you left. after all, you don't want to start from scratch again. You want continuity of care.

Call her office and remind her... and ask whoever answers the phone for the referral number if the therapist is too stupid to contact you. I'd just call them over and over until i got an answer. It's their job to help us

Well technically she is supposed to refer as patient abandonment is serious. But you have to do your part and call and ask for the referral. She may be busy and you are off the radar. The squeaky wheel gets the oil. You dont need to be aggressive about it. Just call and be nice and say "can you please provide me the name of the provider you recommend? If she doesnt call you back or provide the name in a few weeks you can push a little harder.
