Make an appt with a dietician to understand your food restrictions. You need to understand how to read nutrition lables and to understand glycemic levels and then how to apply this information to your diet. In general people with type 2 diabetes are advised to follow a low carb diet, and there are hundreds of books on this subject at your local book store. But does this mean you never ever eat a potato or spaghetti ever again? In addition, you have issues with sodium and fats, so you really need to learn how to make healthy tasty food that will helpl get your BP, cholesterol and blood sugar all in order.
Ask your doc for a referal to see a dietician and you'll be that much closer to getting your health issues under control.

Type 2 diabetes, usually referred to as adult onset, is when your body produces insulin but can not use it properly. This type can be treated successfully with the right diet and exercise in most circumstances. Read here https://tr.im/diabetestreat
If blood sugars are not kept under control at some point insulin will probably be integrated / oral medications are also available. People with diabetes can live long, happy lives but must be diligent in their care and make sure you see your endocrinologist on a regular basis.

Here is a forum where diabetics talk about meals and meal planning. Remember what a lot of people call a "healthy diet" is not healthy for diabetics. http://community.diabetes.org/n/pfx/forum.aspx?web...
