What is the best way to tell a new job that won't be able to work with them anymore?

What is the best way to tell HR, Supervisor, manager,  etc... that I won't be able to come work for them anymore in good manner or politely.

    Simple, direct, non committal, and thank them for the opportunity.  To HR and CC the rest.  ...Include something along the lines of :This is to notify you I will be unable to continue employment at this time and will give 2 weeks notice,  my last working date date is X. Thank you for the opportunity to to (work for you,) etc.  No need to make excuses or lie.

    I regret to inform you that this job totally sucks AND blows. I may have to extend my lunch break into not working here anymore.

    Tell them you have a doctor's note saying you are to be off for X number of days

    Tell the person who makes your schedule ...... Say I need to quit because of....?  And I will happily work a 2 week notice.
