1095-A arrived late, IRS asked for my 8962 it in a letter. They said just send the 8962 and a copy of the 1095-A. DO I need to amend my 1040?

Page two of the 1040 showes an extra penalty tax of $934 (25%) for the $3,737 1099R distribution. Yes I did take it early because I was out of that job and cashed it in to live on. Not for medical or mortgage or house down payment. Is there any way to avoid paying the penalty? I know I pay any income tax due on the $3,737 as well. I am 31 years old and file Head of Household with two 12 yr old dependants.Thank You so much.

When I bit down this morning the 4th tooth from the back (including all molars), on the bottom was the only tooth touching on the bottom row. When I pressed my jaws together the one tooth hurt a little but all my teeth connected again. It has happened a few times in the past few weeks but never this bad. It is a Canadian, with a green card, living in and paying tax to the USA for the past 10 years.

.A 2006 US 1040 tax return is filed In January and a $15,000USD tax is paid to the IRS.

...Now I believe that Canada will want a tax return filed as well.
