  • List of Required Medical Documents?

    I'm having a hard time finding information on a list of required documents that four types of facilities would have. I've tried looking online and nothing seems to be coming up. I've tried CMS.gov with no luck. I need a list of documents that would transverse from paper to electronic health records. The places are a hospital, rehab center, nursing home, home health, and a clinic. I already know every one of them need administrative information (patient name, age, etc.). I also need to know what clinical information systems might be implemented that can be used by all entities?

    There is ads all over my computer?

    Oh, gosh. They are ruining my life. I bought this computer almost a year ago. Brand spanking new from Best Buy. I loved it. It was fast, no ads, I could do anything. A few months ago I was looking for a program for photo editing for a website I was making for a friend. I downloaded one and my computer had a spaz attack. I deleted it instantly and ran my virus scan (which was from Best Buy) and it said that there was two threats removed. I was thinking it was a close call. Ever since then I have ads like crazy! I'm not talking about the usual ads you have on the side bar on websites like Facebook. I'm talking where when I go onto a new site and click, another tab pops up about taking a survey or something. All the time.

    I can't go on Youtube anymore. On the side where they show videos, every third video there is an ad, they talk, play music, etc. They are always changing, which makes the page constantly loading, making it slow. I try muting the ads, but they just change to another one. They also have these ads on websites, such as: Craigslist, Wiki, Yahoo, etc. I run my virus scan religiously (It's Webroot by the way) and it says there is no threats, but there is still ads. Oh, the ads say they are not from that site. So I guess I have either a virus or spyware.

    Question is, what do I do to remove it? When I bought this computer I bought a year warranty for coverage like water damage, drops, one replacement battery, etc. It's up this month. Do you think if I bring it in they can remove this? I thought a virus scan would PREVENT things like this! If they won't, what else can I do to fix this? I've only seen this once before on my mom's laptop and it eventually crashed. I don't want that as I use this computer for my online school. Please help? Thanks!

  • Rat Proofing A Bedroom?

    I have four rats and I allow them to run around my room during free time. I've rat proofed my bedroom, for the most part (picking up wires, making sure there is nothing dangerous they can eat, etc.). There's one thing I don't know how to fix. My bedroom and closet door have a large space under them (about an inch to two inches). At the moment I taped on a piece of the large cardboard papers onto the bottom of my bedroom door to keep them from getting out, but it wouldn't hold if they wanted to. I also tuck a sheet under my closet door to keep them from getting in there. We rent this place, so I can screw or nail anything on to the door, but I want to have something that permanent that I don't have to keep taking on and off when they come out. Any ideas? Thanks!

  • Where could I find a heavy duty bicycle?

    When I was a child, I use to love riding my bike everywhere, I was always on a heavier side, but the bike balanced me out. When I was 11 years old I broke my ankle on my bike, which made me fearful of riding again, during that time I put on a lot of weight, because I wasn't getting the exercise I was use to. Then when I turned fifteen, my father passed away and that's when my weight really sky rocketed. I've tried very hard to lose weight, but normal exercise hurts me or I get sprung ankles from walking or standing too long.

    I thought about getting back to bike riding as it wouldn't be as painful and I would enjoy it a lot more, I could even bring my dog out with me. My sister bought me a Beach Cruiser, which is the bikes I always enjoyed, but my legs, my thighs really, had become too big for me to make the complete circle to peddle. I tried raising the seat, but I couldn't too much, because I wouldn't reach the ground. I'm only 5'6.

    I've thought about investing in a three wheeler, that way I don't have to worry about keeping balancing and it may be possible for me to peddle easier. My only problem is finding a bike that would hold me and wouldn't damage the rims of the tires or whatnot. Is there anyway they make heavy duty bikes or ones for larger people. I'd greatly appreciate any help. Thank you.

  • Question about weird rat behavior?

    I'm a new rat owner, I got the first one Nekko about two weeks ago, he's been a delight and warmed up pretty quick to me. A week after I got Nekko, I got Remmy. Remmy was very scared and even bit me twice, I'm in the process of trust training him at the moment, and he's much better since I first got him. Here's what I'm so confused about...

    Remmy trashes the cage, I call it his own inter decorating. I had a nesting box I made out for a small box for Nekko and had put cut up little piece of towels in it to lay in. He loved it, never had a problem. When Remmy came along, he pulls out the towels every night and moves the box around. He also goes this with the litter box I made for them to use. He's laying in it as I type. This morning I woke up and all of the bedding and poop was dug out of the box and all over the bottom of the cage. I cleaned the cage and put less bedding in the box (although I didn't have much in there in the first place. I also tied the litter box and the nesting box to the cage bars so he can't move them around anymore. He still tries though.

    Another thing is he moves toys around and a little cardboard roll I put in there. He also will throw the toy balls out of the cage door when it's open and bite at the towel I have on the bottom of the cage. I don't know what his deal is, I find it funny, but weird.

    OH! He also really wants to be in my other rats butt, what's up with that? He will chase Nekko around trying to get in his butt to sniff and finally Nekko stops running and Remmy is lifting his tail sniffing his butt and what looks like licking it. He also wants to sniff his butt when he's pooping. Lol.

  • Question about new rat biting?

    Last Sunday I got a medium size rat from Petco (about 2 -4 months old) I was told two different things when getting him. One told me little to know handling for at least 5 days to pervert biting, while another said I should try to interact with him (a little pet or something) every few hours. I wasn't sure what to do, so I kind of went with my own idea on how to get him use to me.

    First I put a cloth in his cage that I had rubbed on me to get my scent, I left him a lone for an hour or two and then every hour or so I would open his cage and make a little sound and offer him some carrot shreds. At first he wouldn't take it and was scared, so I left it in front of him and left him. By the next day he was taking them out of my hand and allowing me to give him a little pet on his back. By the next day I was able to pick him up (using the scooping method, I did a little research before getting him) and putting him on my bed. Before long he was crawling all over me, going into my shirt and hair and sitting the crook of my arm and letting me pet him. No biting what so ever.

    This Friday I went to get him a friend since I was able to get a bigger cage. I went to the store I got Nekko, the first rat, and was planning to get a rat out of the cage he was already in, but I saw another one that was tan and white and so cute. He was a little bigger than Nekko, but not by much. They let me hold him first and he sat in the crook of my arm, just like Nekko. So I picked him.

    When we got home I picked him out of his box and set him in his cage and had dinner. (about thirty minutes or so) then I took him and Nekko and put them in my mom's garden tub to meet. It went fine and they hit it off, so I let them go together in the same cage. They were playing and laying together and all was fine.

    A hour or so later I opened their cage and went to pick up Remy, the new rat, to bring him out to run around with Nekko and he nipped the tip of my finger. He didn't break the skin and it didn't hurt. I realized then that I was handling him way to much for the first day and left him alone. Today I came to the cage and sat in front of it and was feeding Remy and Nekko carrote shreds, one at a time. I had fed them each probably ten when I put the carrot out for Remy (now note, I didn't stick my hand in the cage, because he had come out of their nesting box, it was right in front of the open door.) He came up to me, completely ignored the carrot between my two fingers and sniffed the knuckle and then bit it, hard, drawing blood.

    Why did he do that when I wasn't going for him at all? Now I'm a bit scared of him, because I don't wanna be bit. I'm afraid of him even smelling me. Now all he does is sit on the top floor in the hammock. I put the cage on the floor and while Nekko will come out and run around then run back and so on, he just sits in there. What do I do? I'd greatly appreciate any tips. I don't want an aggressive rat or a biter.

    I just now took the hammock out and rubbed it on me a little to get my scent, so he will be use to it since he is laying in it so much. Here's a photo of them and the cage.

  • Need some information on building a rat cage?

    I have basically all I need, I'm just not sure what kind of wire I should use where 1. the rats can't chew through or fit through and 2. Won't hurt the rats.

  • Liquid coming from my dog's rump?

    Back round information:

    She's a lab/pitbull mix.

    She's 10 months old.

    Fixed.

    50lbs.

    Been to the vet recently for a checkup --No problems apart from a minor skin problem.

    Have had no incidence before this.

    Just changed her food from Natural Choice to Purina One about two weeks ago. (no diarrhea or vomiting)

    This evening my dog climbed onto my bed and fell asleep at my feet. An hour or so later, when I got up to go to the bathroom, she got off the bed as well. I noticed a quite large wet spot where she was laying, At first I thought she pee'd, but she was laying down. I sniffed it and it had no odor. I looked at her rump/private area and there didn't seem to have anything wrong, the bottom of her tail near her butt was wet, so I came to the conclusion that's where it came from. I took her outside and she went to the bathroom and she didn't seem to have any discomfort. Any clue what it could be? I will probably call her vet tomorrow to ask, but I'm curious right now. I've gotten her anal glands done before (it was part of a puppy package at her groomer), but only that once. Could she need it done again?? Thanks!

  • Which CD do you use after you install an expiation pack on Sims 3? (PC obviously)?

    I have Sims 3 and Sims 3 Pets already on my computer. I have to use the Sims 3 Pet's CD to play the game. My friend has the Supernatural expiation pack (I think that's the name, with the witches, vampires, werewolves, etc.) If I download it onto my computer, which CD would I need to use? Is there a site or something that tells you the orders the CD's would have to be played or if you have this you have to use this CD over that. I know it's confusing, not sure how to explain it any easier. lol. Thanks!

  • Can shampoo in the eyes cause discharge?

    When I was taking a shower this morning, a big glop of shampoo fell in my eye and oh, did it hurt! The whole time in the shower I had to keep my eyes close because I couldn't open the one or blink it hurt so bad. While I was in the shower I rinsed it out the best I could with cold water, but it still hurt to blink. After a bit I could open it and see my eye was bloodshot (reasonable response). It still hurt a little to blink, but was fine an hour later. Now I noticed tonight that the one eye is getting yellowish discharge (reminds me of eye boogers you get in the morning, but slimy, not dry) in the corner near my nose. It doesn't hurt, but I've had to wipe some off four times in the past hour. Is this from the shampoo? It was a lot that fell in my eye. I can see out of it fine, no problems. What could it be?

  • Dog growls and barks at you playfully allowed?

    I have a Pit/Lab who's 7 months old. She's very playful and when I look at her and bark or growl she'll growl and bark back while wagging her tail. Is this bad? I know she's playing, but everyone says your dog shouldn't growl as it's a dominance thing. I use to play tug-a-war with her, but I was told with her breed mix I shouldn't, because it's a game of dominance. I'm curious if this is true or just a bunch of stories?

  • Question about my Pit Bull/Lab mix?

    I have a 5 month (on the 14th) old Pit Bull/Lab mix puppy. She's a good dog, although she likes to challenge me a lot, which I am working with her on and she's starting to get the point that she's not the alpha here. I'm curious though. When she's out and about in the house, she likes to get attention from everyone, but when I leave the room, she'll follow me. She goes in my room on my bed with me while I play on the computer she sleeps or chews on her toys. She follows me in the bathroom etc. etc. Why does she follow me so much? She's done it since she was very little. She also only listens to me. She could be going after something and my step father could tell her to "leave it", but it's like she ignores him. But as soon as I say "Mya, leave it!" in a stern voice, she walks away from it. I do most of her training, I feed her, I basically do everything. Which I know is suppose to give you the alpha status with them. But, we also have a dachshund that I do the same for, but she doesn't listen to me, is scared of me (everyone for that matter) except my mom, she follows her everywhere. So, what's the difference?

    Also, Mya is 39 pounds right now at almost 5 months and she's lean, not over weight. I just got her spayed a few days ago, does anyone know how to determine her adult weight? I found a place where you divide the weight she is right now (39lbs) and her age in weeks (21) and times is by 52. The answer was 96lbs. That is HUGE. The vet had said no more that 50lbs - 60lbs...any guesses? Thanks!

  • I need some dog training tips?

    I have a Rottweiler, Pitbull, and Lab mix puppy. She is 12 weeks old as of today. She's very smart and at the age of 9 weeks she already knew: sit, down, paw, gimme ten, speak, and roll over. She's a good dog and very loving, but I'm having a few problems with her that I personally am lost with. BITING is number one. All she does is bite. You're walking, shes biting onto your clothes, you pet her, she's biting your hands, you walk her, she's biting the leash. Her biting is starting to really hurt now with her getting bigger. Just today she jumped up to bite my hands and bit my stomach and it hurt. It's not aggressive biting either, I know she's playing and puppies bite, but I want this under control before it gets way out of hand. She has a lot of toys, but still likes chewing on us instead.

    Another thing she does and is chase my cat none stop. I wouldn't mind them playing if she wasn't so rough. She jumps at him and bins him down and makes him cry. He never jumped on the counters before, but has started to get away from her. She's not trying to hurt, just play but she doesn't know her own strength. There is those few times that they'll lay together and lick each other, but then she's right back to play mode. I've tried a few things with the biting; sour apple, tooth paste, time outs. Nothing seems to work. Another things that isn't as bad as the other problems is her pulling on the leash, especially if she see someone, she wants to go meet them and pulls toward them wagging her tail, she doesn't bark, but not always does someone want to pet her.

    She's a lovely girl and I think she does it on purpose to get to me. When she's doing something wrong she looks right at you too. She's quite a personality. I'm putting her in puppy classes the 18, but not sure if they would help much with some of her problems as they're mostly at home. I can't afford a personal trainer either. Someone suggest a shock collar, my sister had one for her puggle that always ran away when outside and stole food from the table, etc. she used it on him for about two weeks and never had to use it again and he was the best behaved dog I've ever seen. I'd just like some advice because I get so frustrated with her sometimes I don't know what to do.

  • Kennel Cough? PLEASE help!?

    I have to four week puppy (orphaned because her mother abandoned her) and she's been doing great, but recently I noticed she does this hacking sound every once in a while, mostly after she eats (Milk and a little soft puppy starter food) and she has a little runny nose. She's not acting weird and still sleeps, eats, and plays a little as usual so I didn't think anything of it. I'm sitting with her now and she's laying in her knennel and she did the little hacking twice. So, I figured I'd look it up. Could she possibly have Kennel Cough? Also, how can I treat it at home. At the moment I can't take her to the vet so working with her at home is all I can do. I read some things about if you rub their throat and they start to hack, it means they have it. I tried and she didn't do anything. They said you can give them honey to sooth their throat, but I wasn't sure with her age or how much to give her. This won't kill her right? I'd be heart broken. Please let me know and thank you!

  • Orphan puppy Information:?

    Last week my friend brought over a three week Pitbull/Rottweiler puppy. Her mother was young and abandoned them, so she took it home. They couldn't keep her themselves, so I took her in. She's a plump little thing and is healthy; walking around, even has started to howl. I've had her for almost a week now and this is the schedule she and I have set:

    5:00am she wakes me up hungry. I just started introducing her to starter food, only a teaspoon at breakfast, lunch, and dinner. After she eats that I make four ounces of milk (she drinks goat milk, that is what they put her on and the vet told me not to change it because she was already use to it) she only drinks half of it and then we usually go back to bed until 7am or 8am and then she drinks the other half. Then goes to sleep.

    At 12:30pm she wakes up and starts to cry to eat. Once again I feed her a teaspoon of started wet puppy food and the four ounces, but she only drinks two.

    2:00pm she finished the other half.

    6:00pm or 7:00pm she's hungry again and I give her the teaspoon of wet food and she drinks two ounces of milk. Then finally at 11pm or 12am she finishes the other half off.

    This is what she has been doing since I've got her. She's doing really well, goes to the bathroom good, walks around, barks a little and is a little playful but loves to sleep. This monday she will be four weeks and I've been told to move her to three feedings. How would I fix the schedule up so it wouldn't leave her hungry or crying through the night? Also, at 6 weeks I read she should be off the milk and onto dry puppy food mixed with wet until about 9 weeks. Is this correct? So when she gets to 5 weeks, right before I ween her off the milk should I move it to two bottel feedings a day and more wet food?

    Thanks everyone for your help. And please, don't go on about how it's dangerous to have her at this age, and how she should be with her mother--I know. But, as I stated, she was abandoned. I've taken care of a few litters of abandon kittens but not younger than four or five weeks and never a puppy at this age. Thanks for you help.

  • Types of American Pittbull Terriers?

    Mostly known as "Pitbulls", I don't know if this is just slag or what not, but I have heard of the "Red-nosed Pit" (Which they have more of a redder nose) and the "Blue-Nosed Pitt". (I'm guessing their nose is grayish?) How do you determine which branch your Pitbull is in? What's the difference? I've also heard of something called a "Low Rider Pitbull" it's where they have shorter legs than normal Pittbulls so they are closer to the ground. I just got a Pitbull puppy and she is all black except the white on her chest, a thin white strip down her snout, and some of her toes are white. Her nose looks the name color as her black fur. (She's also mixed with Rottweiler I was told but she favors the Pitbull looks) What type of Pitt is she? Also, she is three or four weeks old and I'm bottle feeding her, because her mother stopped taking care of her. How can I tell what her size about would be? Thank you!
